The State of Minnesota requires all children attending school to have received specific vaccinations or to have an exemption form on file with their school district.
To complete your child’s health record, we need either:
- Documentation of all required immunizations or
- Medical or non-medical exemption documentation
To provide immunization records: Please complete the first page of the Minnesota Department of Health Immunization Form included below. You may be able to gain the needed information for the form by calling your child’s primary care provider or downloading your child’s immunization records from their secure website. You can return physical forms using the return envelope included in this packet, or you may choose to send them by email or request a fax be sent from your child’s primary care provider to your site’s school’s health room.
To find immunizations for your child: Visit your child’s primary care provider or contact your local county public health agency.
To provide a non-medical exemption: Please complete the second page of the Minnesota Department of Health Immunization Form included below and have the form signed by a notary. Exemption forms will need to be re-submitted if there is a change in immunization requirements. Notaries are available on-site in all three school and district offices.
